I know there have been some discussion about this. I put in a request for
help and they told me to email DOH. I did that and received a response
today that my information that I sent in for the vaccine probably doesn't
match my info that I"m entering on the website, even though it's my
birthday, gender and address. Seems like the database has an issue.

The person who replied said I could go to the original vaccine site and
verify my info and they could link it or to a participating Healthcare
Provider or Pharmacist who can verify and link.Of course she didn't give me
a list of participating providers.

I know some people have had luck. Has anyone who had an issue resolved
their issue?
